The Condition Management Programme

The Condition Management Programme helps you manage your health condition to allow you to progress towards, move into and stay in employment.

The programme is led by healthcare professionals, such as occupational therapists, physiotherapists and mental health nurses.

It is a well-established, clinically proven, free programme which can offer up to a maximum of 12 sessions of 1:1 and or group intervention.

For more information, follow the link below

When ordering repeat prescriptions, please allow 48 hours before collecting from your chosen chemist, so your request can be fully processed.

We will only look into any routine queries regarding prescription issues, after 48 hours has passed since the order was placed.

Thank you for your understanding in this matter.
